The forum is titled “Life and Death With Dignity: Taking Charge of Health Care Decisions.”

“If you became too sick to convey your wishes, would your family, friends or doctors know what kind of medical care you want?” Amy McNamara, League of Women Voters of New Castle health care committee chairwoman, said in a press release.

“Do you struggle with knowing what end-of-life health care options are available for family members?

“Planning ahead can reduce stress and allow greater autonomy when making end-of-life decisions for your family or yourself.”

The Rev. Martha Jacobs, former hospital chaplain and senior minister at First Congregational Church of Chappaqua, will serve as the moderator.

Panelists will include doctors and nurses from area hospitals and hospices, as well as attorneys from the Elder Justice Unit at the Pace Women’s Justice Center.

The event, which includes refreshments, will be 7-9 p.m. in the theater at the Chappaqua Library, 195 S. Greeley Ave.
